Hemodialysis therapy

The patient's blood and dialysate are introduced into the dialyzer at the same time (the flow direction of both is opposite), the semi permeable membrane of the dialyzer (artificial kidney) is used to clear out the excessive toxin and excessive water accumulated in the blood, and the base is supplemented to correct acidosis, adjust electrolyte disorder and replace the excretory function of the kidney.

Hemodialyzer, commonly known as artificial kidney, has three types: hollow fiber type, coil type and plate type. The most commonly used is hollow fiber type, which is composed of 10000 ~ 15000 hollow fibers. The wall of hollow fiber is dialysis membrane, which has the property of semi permeable membrane. During hemodialysis, blood flows into each hollow fiber, while dialysate flows outside each hollow fiber. The flow direction of blood is opposite to that of dialysate. Poisons are removed through the principle of semi permeable membrane, and water is removed through ultrafiltration and infiltration.

Indications and contraindications

The indications of hemodialysis include: ① acute renal failure. ② Acute drug or poison poisoning. ③ Chronic renal failure. ④ Renal failure before renal transplantation or abnormal reaction after renal transplantation make the transplanted kidney nonfunctional. ⑤ Other diseases (liver failure, schizophrenia, psoriasis, etc.).

The relative contraindications of hemodialysis include: ① patients with extremely critical condition, hypotension and shock. ② Severe septicemia. ③ Severe myocardial dysfunction or coronary heart disease. ④ Within 3 days after major operation. ⑤ Severe bleeding tendency, cerebral hemorrhage and severe anemia. ⑥ Psychotic uncooperative. ⑦ Patients with malignant tumors.

Dialysis

General patients need hemodialysis 3 times a week, 4 ~ 5 hours each time. Dialysis should be started as soon as possible to correct the irreversible organ damage and metabolic disorder caused by excessive toxin accumulation. Dialysis should be started when the creatinine clearance rate decreases to 10 ~ 12ml / min.
